Athletics > Basketball I must have the registration form and fee before the deadline to guarantee your child’s participation this season. The program filing deadline with CYO is the following week. I am unable to add teams once the CYO deadline passes. Team selections will begin the week of October 27th or the week of November 3rd. The CYO Athletic Contract (form available on CYO website www.cyss.org) must be completed prior to the first game. This form requires a signature by a physician, advanced nurse practitioner or physician’s assistant. If this form was turned in for you child’s participation in a fall sport, another contract is NOT required. The emergency medical authorization form (available on CYO website www.cyss.org) must also be completed. A new form must be completed for basketball (even though you may have completed one for your child’s participation in a fall sport.) How is your $50 fee spent? The majority of your basketball fee is used to pay the CYO team entry fee ($150 per team) and CYO player fees ($25 per player). Remaining monies are used to purchase uniforms, basketballs, and other equipment.
